Please complete the following form to submit a re-installation request for one or more stations or server. Once received, a technician will contact you to schedule the installation.

Note: Please make sure all servers/stations are ready to be installed at the time of the installation. This includes having all Printers and Scanners installed and operational, as well as all Windows Updates. You will also need an administrative account to the computer.
